Health Department

Community Health and Wellness Team

HOME

The team seeks to reduce the risk of disease and early death in the community by raising awareness of and encouraging participation in physical activity and healthy eating. The Metro Public Health Department is the lead agency for the “Nashville Community Health and Wellness Team”, a community coalition for which the Health Promotion and Chronic Disease Prevention division provides coordination and support. The coalition is made up of representatives from hospitals, agencies, businesses, advocacy groups, and concerned citizens. During 2003, the team was incorporated as a not-for-profit charitable organization, and has been certified as 501(c)(3) by the Internal Revenue Service. This means that all charitable contributions toward the team’s work are tax deductible. The team is eligible to apply for grants from foundations which require 501(c)(3) status.

VISION

The team’s vision is “A Nashville where everyone has the access to healthy nutrition and physical activity in the built environment”.

MISSION

The mission is to “empower the Nashville area to be active and eat healthy”.
