Kidney Disease Prevention
Providing Education and Behavior Goal Coaching
The Kidney Disease Prevention Program provides education and telephonic case management to individuals at risk for Chronic Kidney Disease due to diabetes and/or hypertension.
A Registered Nurse, Certified Diabetes Educator provides behavior goal coaching and educational services to clients in order to assist them with achieving better control of their blood sugar and blood pressure.
Program Services include:
- Individual or group diabetes education
- Behavior goal coaching through telephonic appointments
