Position Description:
Under the direction of the Director of Nursing or designated supervisor this Population-based Public Health Nurse, located in the community, will demonstrate application of public health nursing concepts in nursing actions with individuals, families, and community/special populations.
Actively engages the community to collect data required for community assessment.
Analyzes data to determine nursing diagnoses that guide health promotion, illness reduction, and rehabilitation.
Develops specific community-based interventions that are culturally sensitive and linguistically appropriate.
Implements the plans to achieve health goals and objectives.
Evaluates responses to interventions to identify progress toward goals and to revise as needed.
Utilizes evaluation results to propose program changes and influence policy.
Fosters the development of community action groups such as coalitions.
Collaborates with other disciplines internally and externally.
Mentors and supervises students and interns.
Minimum Requirements:
Bachelor’s degree in Nursing with 3 years of public health experience. Master’s degree in public health or nursing is preferred.
The ideal candidate will possess excellent written and oral communication skills, strong strategic leadership skills, sound knowledge of population based public health practice, experience with building and sustaining community coalitions and ability to work collegially with diverse people. This individual must practice with a high level of initiative and serve as a community organizer.
